Dear Valued Patients,

After 40+ years of practice I have decided to retire for personal reasons. Therefore, I am transferring my practice to Dr Vincent Ostrowski, Midwest Ear Institute. This becomes effective April 1, 2020. Dr Ostrowski will have access to your medical records and is accepting new patients and may be available to assume your care. Please make an appointment to see him. If you prefer, you may contact your medical plan/health insurance company to obtain a list of physicians in the area. You may also contact the local medical society or the physician referral service of your hospital of choice for the names of physicians in your area.

If you seek care elsewhere, I will forward a copy of your medical record to your new physician upon receipt of your written authorization. You may also pick them up at East office location: 1400 No. Ritter Ave., Ste 221, Indianapolis, IN 46219. An authorization form is enclosed for your convenience. Please call in advance if coming to pick up records. The office will close 3-27-20.
